Affirmative’s mission is to make care navigation services affordable and accessible to all members of the trans community. Our team will offer you the personal attention and support you deserve while using technology to quickly and effectively find the right care providers to meet your needs.

What is care navigation?
Care navigators work with you to connect you to the medical resources you need. Looking for a therapist who will take medicaid? Trying to find the right marketplace plan to cover a gender-affirming surgery? Just need help scheduling some follow-up appointments? We’ll take care of it.

How will it work?
You’ll sign up below and answer a few questions. If we think we might be able to help, we’ll reach out to you via the email you leave with us. You’ll have a quick call with a care navigator to discuss your goals. Then, we’ll work to get you everything you need. You may be asked to provide feedback on your experience.
Will Affirmative always be free?
In order to pay our navigators a living wage, we’ll be charging users who are able to pay a monthly fee of $25 once the platform goes fully live. We’ll also work with community health organizations to make sure everyone–regardless of their personal ability to pay–has access to the care navigation services they deserve.
No pilot participants will be charged during our pilot, and signing up for our waitlist does NOT mean you’ve signed up for a paid service.

Who is eligible?
